The Franklin Regional Band includes Concert Band, Wind Ensemble, Jazz Band, and Marching Band. The band is part of the Franklin Regional School District located in Murrysville, Pennsylvania and is under the direction of Mr. Kevin Pollock.
 

IMPORTANT INFORMATION ABOUT THE EMERGENCY MEDICAL HEALTH FORM

Click on the link below to open the Emergency Medical Health Form. TYPE all of the information into the form. Please do not print the form and write in all of the information.  Typing will make this important information much easier to read when needed.

Once you have completed the form, press the button at the bottom to print the form.  Attach a copy of both sides of your child’s medical insurance card(s) to the form.  Drop off everything at the high school band office.

Once you print the form, please check the form for completeness. If some of the form is truncated, adjust your printer settings and reprint the form.  Some printer drivers come with a “Shrink To Fit” option which makes this process easy.
